Genetically engineered compound for back pain falls short

ScienceDaily (Apr. 16, 2012) ? Despite the great promise that injecting a new type of anti-inflammatory pain medicine into the spine could relieve the severe leg and lower back pain of sciatica, a Johns Hopkins-led study has found that the current standard of care with steroid injections still does better.

Etanercept, sold by the brand-name Enbrel, is a genetically engineered small-protein drug known as a tumor necrosis factor inhibitor (TNF). Currently, it is used to treat rheumatoid arthritis and other autoimmune disorders in which the immune system attacks healthy tissue causing pain, swelling and damage. The drug blocks TNF, a naturally produced substance that causes inflammation.

Although steroids combat inflammation generally, TNF inhibitors specifically target the inflammatory molecules causing sciatica and other pain conditions, and keep them from binding to receptors in the central nervous system, potentially preventing the pain altogether.

A team led by Steven P. Cohen, M.D., an associate professor of anesthesiology and critical care medicine at the Johns Hopkins University School of Medicine, conducted a blinded, placebo-controlled study, providing epidural injections of either 60 milligrams of a steroid, 4 milligrams of etanercept, or 2 milliliters of saline to 84 adult patients with sciatica. One month after the second of two injections, the patients on steroids reported less pain and less disability than those in the other two groups. The study is published in the April 17 issue of the Annals of Internal Medicine.

Cohen said studies with etanercept grew out of efforts to prevent or limit the pain that commonly comes from a herniated disc pressing on a nerve root in the lower back or neck. Steroids work, he said, but they are not without drawbacks, including mixed and only temporary results in relieving pain, and the potential for catastrophic complications. Pain experts have long been working to try to find an alternative treatment that is safe and reliable, he notes.

“People are desperate for a safer, more effective drug,” Cohen says. “This new treatment shows a lot of promise, but at least in the doses we gave it — the dose known to be safe — steroids still work better. And in those lower doses, etanercept may not be the drug everyone’s hoping it is. There’s still a lot more work to be done.”

Cohen says a study published last month found that patients who received more than twice as much etanercept as was used in his study felt better than those who got steroids one and two weeks after their injections, but not four weeks out. The dose of etanercept administered by Cohen’s team — 4 milligrams — was deemed to be the optimum dose found to be both safe and effective in a pilot study done by Cohen and colleagues in patients and animals.

The idea behind administering an epidural injection for sciatica is to bathe the spinal nerve roots directly in a medication designed to reduce inflammation — and pain — in order to give the body time to heal itself, Cohen says. This allows for better relief at lower doses and, in turn, fewer side effects than when the medications are given by mouth or intravenously.

The new study found that more patients treated with epidural steroids (75 percent) reported 50 percent or greater leg pain relief and felt better overall after one month compared to those who received saline (50 percent) or etanercept (42 percent). On a pain scale of 0 to 10, with 10 denoting the worst pain, those who received steroids reported, at one month, an average pain score of 2.1 compared with 3.6 in the etanercept group and 3.8 in the group injected with saline. Those in the steroid group also reported lower levels of disability (21 percent) than those in the saline group (29 percent) or etanercept group (38 percent).

After six months, however, slightly more patients in the saline (40 percent) and etanercept (38 percent) groups had a positive outcome than those in the steroid group (29 percent).

“The effect of the steroids didn’t last,” Cohen says, “affirming the fact that steroids work, but not for very long.”

Cohen says research now needs to be done to test the safety and effectiveness of higher doses of etanercept and other drugs that block the inflammatory molecules responsible for pain.

Funding for the study was provided by the John P. Murtha Neuroscience and Pain Institute, the International Spinal Intervention Society and the Center for Rehabilitation Sciences Research.

HOBOKEN, N.J. (AP) ? There is no snooze button. If you unplug it, a battery takes over. As wake-up time approaches, you cannot reset the alarm time.

It could be the world’s most exasperating alarm clock.

Once it goes off, to stop it you must get out of bed, go into the kitchen or bathroom, and punch the day’s date into a telephone-style keypad. That’s the only way to stop the loud ‘ding-ding,’ designed to sound like a customer angrily banging on a concierge bell at a hotel.

It was invented by Paul Sammut, a 25-year-old engineer who lives in Hoboken. During the day, he builds and researches underwater robots and vehicles at the nearby Stevens Institute of Technology.

He started working on the gadget because he was finding it hard to get up and make it to work on time after college.

“I wanted to make something that would essentially force me to get out of bed when I wanted to get out of bed the night before,” said Sammut. “And I was thinking about ways of doing it and I thought about how in high school I had the perfect solution to this, which was my mother, and how she would, if it was time for me to wake up, she would force me out of bed.”

He built the prototype in his spare time and uses it every day.

“Now I wake up before it goes off,” said Sammut. “I subconsciously fear it and know I have to get up.”

After a friend suggested he try and sell the device, he made a video demonstrating it, and posted that on kickstarter.com. That’s a website where the general public can support creative ideas by investing in them financially.

“We raised over $150,000 over a month and a half and we currently have over 400 orders,” said Sammut.

Sammut has formed a company and is now trying to fill all the clock orders by the end of the summer.

He acknowledges there is one way to stop the alarm without getting out of bed.

“You could smash it,” said Sammut.

But with a $350 price tag, that would be a really expensive way to sleep in.

Most people are about 90% satisfied with their home. For that small 10% though the urge to make adjustments and changes to their property have some homeowners doing projects that can make their house more appealing and livable. Home improvement projects often start out in one room and spread throughout the house. Working to make a house more livable for the family and meet the demands of each individual, the renovation projects that are contracted by professionals allow a homeowner to have exactly what they want from their house when all the work is done. By starting out small and doing simple improvements many homeowners are able to manage their schedule of changes and improvements throughout their home.

One way to start out making changes to the exterior of the house without disrupting the daily routine of the family is to install replacement windows. San Diego homeowners that are looking to increase their energy efficiency and lower their air conditioning costs have found that by using vinyl windows, San Diego property values have increased somewhat. With low E windows that have a high R value the resale market for homes has shown improvement. But most people just enjoy the benefit of having their house be more energy efficient with their window upgrade.

Once the windows have been brought in a number of homeowners chose to continue their treatment of the house by purchasing complimenting sliding glass doors that are matched to the vinyl windows and also provide a cost savings on the utility bills. With greater efficiency inside their dwelling many people go on to add new lighting fixtures and other small touches to their homes that can enhance the style of their living space.

For those that are more aggressive in their renovation strategy, the tearing down of walls and remodeling of the kitchen leads them to change out the floor plan of their house and create a space that is perfect for entertaining guest and preparing meals. With an addition to the kitchen the opportunity to add new windows that allow more light into the home or a set of stylish French doors give San Diego homeowners a more appealing place to live that is customized to their standards. Whether it is just making a home more energy efficient or doing a few renovations within the property many San Diego homeowners are finding that with the right touches to their windows and doors they have a beautiful house that looks great from the neighborhood and is practical and functional for them and their family.

Cancer Treatment: Maintenance Rituxan for CLL.

Rituximab has been shown to prolong survival when used therapeutically with chemotherapy in CLL. This Rituximab, the mAb targeting CD20, was approved by the US FDA for patients with relapsed low-grade non-Hodgkin lymphoma. Relatively low levels of CD20 are expressed on CLL B cells, compared to normal B or neoplastic B cells of other lymphomas. In addition, soluble CD20 has been demonstrated in plasma of patients with CLL; this may inhibit the capacity of rituximab to bind to CLL B cells, thereby resulting in rapid clearance and negatively affecting pharmacokinetics. Standard-dose rituximab (375 mg/m2 weekly for 4 weeks) has very limited activity for patients with CLL. Dose-intense and dose-dense single-agent rituximab has been shown to increase efficacy but remains experimental. Rituxan at standard dose? is? listed by NCCN as a single agent only for frail patients who cannot tolerate other treatments. A 2005 guideline (Imre et al) says: “There is currently insufficient evidence to support or refute the additional use of rituximab as a maintenance therapy in patients who have completed chemotherapy plus rituximab.” However, NCCN doest list it as a single agent for the elderly.A recent guideline (Imre et al) says: “There is currently insufficient evidence to support or refute the additional use of rituximab as a maintenance therapy in patients who have completed chemotherapy plus rituximab.”

Since this guideline, additional evidence has accrued to support rituximab for maintenance in CLL. The strongest evidence in support of it was the Primary Rituximab and Maintenance (PRIMA) Study, in which patients received immunochemotherapy. Those who did not fail were randomly assigned to maintenance or observation. There was a longer progression-free survival in those who received rituximab maintenance, but no survival benefit. There also was the? Ardeshna study, in which patients who could be observed were either randomly assigned to the watch-and-wait approach or to rituximab induction and maintenance. Time to requiring next treatment was longer if you got rituximab, but there was no survival benefit.

Currently(2012) are no guidelines that recommend maintenance rituximab.

The IMiDs, thalidomide and lenalidomide, have also been investigated as maintenance treatment in CLL. Lenalidomide might have some validity as maintenance treatment and clinical trials of lenalidomide in maintenance treatment have been initiated.
